The following documents are required for registration:
- Certified birth certificate
- 3 proofs of residency showing physical address (example: current utility bill, telephone bill, etc.)
- Up-to-date immunization record
- Social security card (student and parent)
- Custody/legal documents (if applicable)
- Record indicating grade placement (if applicable)
- Copy of Home Study application (if applicable)
- Medicaid number (if applicable)
- All entering 6th grade students must have the required immunizations to begin school - this is a new state law requirement.
